Diabetes Management Program

About the Program

  • Diabetes management education classes
    You will receive 10 sessions with a Certified Diabetes Educator to help you learn more about diabetes, such as how to check your blood glucose (blood sugar) levels, and what you can do to prevent serious complications.
  • Generic co-payment on most diabetes medications1
    Refer to the UHA Preferred Drug Listing to see if your medication is covered.
  • No co-payment on supplies

To Enroll/Participate

  • Members who are screened in a worksite wellness program, with test results that indicate they have diabetes, are selected and invited to participate.
  • If you have not participated in an onsite screening, but are interested in taking advantage of this program, or for any questions, contact Health Care Services.

Eligibility and Fees

  • All members are eligible
  • No fees if seeing a participating "diabetes management program" provider
  • There may be a co-payment on some medications¹
